Welcome: three steps before we start
Step 1: sign the consent form on your phone
Maine's marketplace and federal rules require your written permission before I look up plans or help with an application. Open the form from your welcome email (or download it below), tap the boxes to fill it, tick what you agree to in section 3, sign section 7 with your finger, and email it to christopheryork@maineunitedhealth.com. Nothing to print, no account to create. iPhone: open the PDF, tap the pen icon, choose Signature. Android or computer: the free Adobe Acrobat Reader, Fill & Sign. A photo of a signed paper copy works too.
Step 2: have these ready for our appointment
Please do not email these; bring them or we go through them on the phone: dates of birth and Social Security numbers for everyone applying; your best estimate of 2027 household income (a recent pay stub or last year's return helps); any coverage you have now and any offered by an employer; your CoverME.gov login if you already have one (I never log in for you).
Step 3: tell me you are coming
Use the short form below with your name, email, phone and the best time to reach you, and say whether you have sent the signed form. Once I have it, I compare every plan available to you and send a short summary of the two or three that fit best, with the price after savings. We pick one together, and you review the application before it is submitted.
